Terms, Acknowledgement and Release
Westlake-Bay Village Rotary Art Festival
Saturday, July 26, 2025 - 10:00 a.m. to 4:00 p.m.
Located at: 27555 Center Ridge Road, Westlake, Ohio 44145

In consideration of acceptance by the Westlake-Bay Village Rotary Club as an exhibitor at the 2025 Westlake-Bay Village Rotary Arts Festival, I hereby agree to the following terms, rules and releases:

● I do hereby release, hold harmless and forever discharge the Westlake-Bay Village Rotary Club from any and all actions, claims and demands for any damage, loss or injury of whatsoever kind of nature, arising from, and by reason of any and all known and unknown, foreseen and unforeseen bodily and personal injuries, damage to any property, including but not limited to, any equipment, personal property or any items in storage, and the consequence thereof, incurred by me, the above exhibitor and assigns of same, resulting from participation Westlake-Bay Village Rotary Arts Festival.

● I further agree to give a right of indemnification to the Westlake-Bay Village Rotary Club for any such occurrences, injuries, losses or damages incurred during the period of use and occupancy of the premises by me or my organization.

● I further acknowledge that there is a $95.00 booth fee ($125, after May 15th). This fee will be refunded to the artist if not accepted into the art festival.

● I further acknowledge that all exhibitors will be responsible for taking care of his/her own sales receipts, collecting sales tax (8.0%) and obtaining a current Ohio Transient Vendor’s License (www.tax.ohio.gov)

● I further acknowledge that exhibitors must carry their own insurance to cover the value of their work. The Westlake-Bay Village Rotary Club assumes no liability for damage, theft or injury on the premises.

● I further acknowledge that exhibitor must be present at all times during the event. Both space is limited to 10’ x 10’. Participants are required to provide their own tent, tables, chairs, displays, etc. Participants should be prepared to secure their tent and displays in the case of wind and/or rain.

● I further acknowledge that the art committee reserve the right to require removal of items they judge to be inappropriate. All paintings, drawings, graphics must be framed, matted or displayed tastefully.

● I further acknowledge that all work exhibited must be original, designed and produce by, the exhibiting artist. No kits, moldware, work produced from commercial patterns, manufactured pieces or BUY-SELL art.

● I further acknowledge receipt of a copy of this Terms, Acknowledgement and Release for participation in the Westlake-Bay Village Rotary Arts Festival.

● An artist’s raffle donation with a minimum value of $35 is required and will be collected the morning of the event.

● I, the undersigned artist, hereby agree to grant Westlake Bay Village Rotary Art Festival the irrevocable, non-exclusive, worldwide right to use my company name, any pictures of my artwork, and the name of my social media channels in any publications, including but not limited to, marketing materials, website content, and social media posts. I understand that this grant of rights includes the right to reproduce, modify, distribute, and display my company name, artwork images, and social media channel names in any form or medium.
